State-Specific Steps

    Book an Appointment With Leafwell

    Speak to a Leafwell healthcare provider to get approved for a medical card in New Jersey.

    During your appointment, your provider will ask you questions based on your medical history to determine if a medical card is a good choice for you.

    You’ll get your money back if you’re not approved.

    Get Approved

    Once approved, you’ll receive a medical certification from Leafwell.

    This certificate says your doctor recommends you become a medical patient and is required by the State of New Jersey to apply for your card.

    Complete an Application With the State of New Jersey

    Register on the New Jersey Medical Program website.

    Once registered, you must complete the medical card application, submit your certification, and pay the state fee.

    Currently, New Jersey charges $10 to process your medical card application.

    Get Your Medical Card Via U.S. Mail

    If approved, the state of New Jersey will send a physical medical card to your home address via U.S. mail within seven to 10 business days.

    • Intractable Skeletal-Muscular Spasticity
    • Irritable Bowel Diseases (IBDs, e.g. Ulcerative Colitis)
    • Migraine
    • Multiple Sclerosis (MS)
    • Muscular Dystrophy (MD)
    • Nausea/Vomiting
    • Severe Opioid Dependence
    • Post-Traumatic Stress Disorder (PTSD, if resistant to conventional therapy)
    • Seizure Disorders, including Epilepsy (if resistant to conventional therapy)
    • Terminal Illness (If the physician has determined a prognosis of less than 12 months of life)
