    During your appointment, your healthcare provider will ask you questions based on your medical history to determine if a medical card is a good choice for you.

    The consultation costs $99. You’ll get your money back if you’re not approved.

    If approved, your Leafwell healthcare provider will contact your designated pharmacy to complete your registration as an LA medical patient.
