    Register for the state website by attaching yourĀ  PDF medical card certificate to a completed State of Arkansas application. You can submit your application online.

    • Severe and persistent muscle spasms including without limitation those characteristic of multiple sclerosis
    • Touretteā€™s syndrome
    • Ulcerative colitis
    • Any other medical condition or its treatment approved by the Department of Health
